Initial Consultation

New patients need to have an initial consultation with Dr. Fakheri prior to treatment. Should you decide to become a patient based on your in-depth consultation.

Prior to your initial consultation you receive a welcome package by mail or electronically containing practice information and forms. To save time, we suggest that you complete the forms and mail or fax them to our office prior to your appointment.
During your initial consultation, Dr. Fakheri will perform a comprehensive periodontal examination and oral cancer screening and he will review all findings with you. At the conclusion of the visit Dr. Fakheri will present you with all different treatment options and you will be explained about all the costs that are involved. No unexpected fees with us! We want to make sure that you fully understand your treatment so Dr. Fakheri will dedicate plenty of time to address any questions or concerns you may have. In some instances we will need more information such as X-rays or 3- dimentional X-rays to accurately plan your treatment. These may be taken or ordered at this visit as well.
We believe that in order to achieve the best results for our patient your periodontist and your dentist should work very closely with each other. That is why we Dr. Fakheri will discuss all findings with your dentist and together, they will create the treatment plan that best suits your needs.
Please allow approximately one hour for your initial consultation. We respect your time and ask that you reciprocate by being on time, even a few minutes early.
